Paying attention to tongue diagnosis in exogenous diseases, and attention to pulse diagnosis in internal diseases
Combination analysis of tongue diagnoses with pulse diagnoses is the most distinctive aspects in four diagnoses. In exogenous emergency, the doctor will pay more attention to tongue, and in internal diseases, more attention to pulse, because the tongue and tongue coating change more quickly, obviously and earlier. In diseases caused by six exogenous pathogenic factors and epidemic diseases, tongue diagnoses have more clinical guidance significance. In this aspect, the specialists of warm school have accumulated abundant experiences. In tongue proper, red tongue margin and tip indicate warm-heat attacks body surface; red tongue indicates pathogen heat enters qi fen from exterior into interior; deep red tongue indicates pathogen heat enters ying fen from qi fen; purplish tongue indicates pathogen heat enters from ying fen to xue fen. The tongue coating changing from white to yellow indicates light heat; from yellow to grey indicates severe heat; from grey to black indicates extreme heat. Besides, in warm diseases, the doctor will touch the tongue to learn cold or heat, moist or dryness (body fluid). Dry white coating indicates deficient stomach yin; dry red coating indicates deficient kidney yin.
In internal diseases, tongue coating change is reference. For example, for the diabetes patients with greasy coating, the doctor will use herbs tonifying yin to moisten dryness but with smaller dose; or add Huoxiang, Peilan, Baidoukou, Cangzhu, Chenpi with small dose; considering greasy coating, herbs eliminating dampness like Banxia and Houpo with large dose is not proper, even leading to aggravation of diseases.
For those with chronic diseases, or old weak patients, comparing tongue diagnoses with pulse diagnoses, the later will be more meaningful, at the same time, heavy pressing pulse is more meaningful than light pressing and reflecting real diseases. For example, a patient with chronic diseases, manifested as red face, vexation, much sweating, thin, weak and powerless pulse, these symptoms indicate yin deficiency and fire, in fact, this case is qi and yin deficiency, bitter cold herbs for purging fire will be avoided, it will be better to select sweet warm herbs for tonifying qi and yin. If the doctor does not feel pulse, only clear deficient fire with herbs tonifying yin and clearing fire, qi deficiency will be aggravated.
